Service Description

For individuals and families managing multiple health conditions, complex needs, or difficulty accessing services, our care management program provides dedicated, personalized coordination. A care manager works one-on-one with each client to develop a care plan, connect them to the right providers, monitor their health and wellbeing, assist with appointments and referrals, and ensure that nothing falls through the cracks. For our Karen, Burmese, and refugee clients, this often means much more — helping them understand diagnoses delivered in a foreign language, communicating with doctors on their behalf, and ensuring their cultural values are respected throughout their care. Care management is the thread that holds every other service together.
